Required Supporting Documentation

Everything You Need to Collect for Your Loan

Thank you for submitting your loan application and for your patience during this process. 

We have begun processing your application and will need you to provide the following supporting documentation to continue the process.  If you have already submitted the items below, disregard this message. 

Please email the following items directly to jeremiah@mountainbizworks.org or use a secure cloud storage service such as Google Drive, Dropbox, or Box to upload the documents and share a link to them:

1. 1. Copy of current ID (driver’s license or passport)
   2. 2019 personal tax return
      • If not available, 
        • 2018 personal tax return
   3. 2019 business tax return
      • If not available,
        • 2018 business tax return AND,
        • 2019 profit & loss
   4. Three most recent business bank statements
   5. COVID-19 Impact Statement  – Please provide a brief statement of the impact COVID-19 has had on your business. This includes, the date you had to close your business, estimated loss of revenues, loss of contracts, cancellations, etc. 

If there is a Co-Applicant involved, we will also need the following information for each Co-Applicant:

• Name 
• Email Address
• Ownership Percentage
• Social Security Number 
• Primary Physical Address
• Copy of current ID (driver’s license or passport)

We’d like to stress that the sooner you can get us the above items, the sooner we can complete the review of your application.

If we do not hear from you within 7 days, your application will be withdrawn.
