PPP Forgiveness Process


Welcome to our online PPP forgiveness application site. We were very pleased to be able to assist with your PPP loan and now we look forward to being able to assist with processing your PPP loan forgiveness application.

Here’s how’s the forgiveness process works…

1) Fully use your PPP loan
Before you apply to have your loan forgiven, make sure that you’ve fully utilized your PPP loan proceeds and can document how you applied those proceeds towards eligible expenditures. Please refer to SBA’s PPP loan forgiveness page for full details.

2) Chose and complete the right SBA Forgiveness Application Form
SBA has published three different application forms. Use the table below to find the one that’s right for you:

3508S Loans $150k or less Download Form
3508EZ Loans over $150k and with no reduction in employee headcount or compensation Download Form
3508 All others Download Form

Review the form instructions carefully to make sure you have the right form for your situation and understand the supporting documentation required.

If possible, we request that you complete and sign your form digitally, however we can also accept forms filled out by hand and then digitally scanned or photographed.

3) Organize the Required Supporting Documents
A) If you are using Form 3508EZ or 3508, you will need to provide documentation to verify your payroll and non-payroll costs along with the completed SBA forgiveness form. Details on exactly what you need to provide is included in the form instructions. Please gather these as you’ll need to upload them.
B) If you are submitting a Second Draw forgiveness application and have not yet submitted documents verifying your 25% revenue reduction, you are also required to provide these at this time. Suitable Documentation may include relevant tax forms, including annual tax forms, or, if relevant tax forms are not available, quarterly financial statements or bank statements. 

4) Submit Your Application via our Online Portal
Use your unique application link we sent you via email to open our online portal and submit your application. If you can’t locate your link, just email as pppf@mountainbizworks.org and we’ll get you another one. Once we’ve received your application, we’ll process it, then we’ll submit it to SBA for their confirmation. Please allow two weeks for a response (we hope, but it could be longer depending on SBA’s additional processing time).
